Reporting a New Injury

CALIFORNIA CLAIMS REPORTING:

As soon as possible, please file your First Notice of Loss. Online Reporting is the fastest and easiest way to report a claim. Click here to Report a Claim Online

Tips for making Online Claim Reporting even easier:

  1. The first time you report an injury online, you will need to have your company tax ID
    number.

  2. After the initial reporting, you will be provided a password to use for subsequent reports of injuries. You can change the password anytime.
  3. Complete the online questionnaire. The questionnaire will ask you familiar questions, they follow the form 5020 (Employer’s First Report of Injury) questions which you have completed in the past.
  4. A complete copy of the 5020 Form will be sent to you via e-mail, along with a letter
    advising you of the claim number and the contact information of your customer service examiner.
