Reporting a New Injury
CALIFORNIA CLAIMS REPORTING:
As soon as possible, please file your First Notice of Loss. Online Reporting is the fastest and easiest way to report a claim. Click here to Report a Claim Online
Tips for making Online Claim Reporting even easier:
- The first time you report an injury online, you will need to have your company tax ID
number.
- After the initial reporting, you will be provided a password to use for subsequent reports of injuries. You can change the password anytime.
- Complete the online questionnaire. The questionnaire will ask you familiar questions, they follow the form 5020 (Employer’s First Report of Injury) questions which you have completed in the past.
- A complete copy of the 5020 Form will be sent to you via e-mail, along with a letter
advising you of the claim number and the contact information of your customer service examiner.
